Through the Active at Home program support workers help older people regain mobility and confidence on their feet, using simple resistance and weight bearing exercises to increase their strength, balance and stamina.
Active at Home is proven to make a real difference, improving the health and overall wellbeing of older people receiving in home care services.
It is designed to be introduced at an earlier stage than other short-term health interventions to help older people protect their mobility and so maintain their independence.
Cost Effective
Active at Home is offered as a cost-effective solution to home care providers, enabling them to make a real difference to the health and wellbeing of their ageing clients, while also meeting Government requirements to deliver (and report on) wellness and reablement services.
Evidence Based Program
Active at Home is an evidence-based program with measured outcomes. It is developed in association with the Australian Government and strongly aligned with the aims of national aged care reforms.
A trial of the program proved it made a significant difference (up to 20%) to clients’ physical performance – the sort of improvement which can mean the difference between older people remaining physically independent and needing more care.
